Conventional wisdom puts a state’s healthcare capacity in its biggest cities. Kentucky’s addiction-treatment map breaks that pattern. Louisville (metro population ~1.28 million) ranks third in facility count, while Corbin — a small city in Whitley/Knox county at the junction of I-75 and the Cumberland Gap region — lists 68 facilities, the second-most in the state. Facilities listed with a Corbin address serve a wide southeastern-Kentucky catchment, not just the town itself.
The pattern tracks the geography of the opioid and, later, fentanyl crisis. Appalachian Kentucky has carried some of the nation’s highest overdose burdens for two decades, and treatment infrastructure — methadone and buprenorphine clinics, residential programs, and outpatient counseling — expanded in response. The result is a treatment network whose center of gravity sits in the mountains, not on the Ohio River.
The flip side of regional hubs is uneven access. Of the 91 Kentucky cities with at least one listed facility, 33 (36%) have only one. A handful of hub cities — Lexington, Corbin, Louisville and Bowling Green — account for a large share of the state’s capacity, while many rural counties depend on driving to a regional center or on telehealth and outpatient MAT.
